The sebaceous gland—the tiny structure responsible for producing oil in the skin—is not something to suppress or eliminate. In fact, it is one of the most important regulators of skin health. Researchers now recognize the sebaceous gland as a key player in maintaining barrier function, immune balance, and inflammation control within the skin.

The real role of the sebaceous gland
Sebaceous glands are located throughout the skin, with the highest concentration on the face, scalp, chest, and upper back. Their primary role is producing sebum—a complex mixture of lipids that includes triglycerides, wax esters, squalene, cholesterol, and free fatty acids.
While sebum is often blamed for acne or oily skin, its purpose is far more important. Healthy sebum helps the skin perform several essential functions:
- Maintaining the integrity of the skin barrier
- Preventing excessive transepidermal water loss
- Supporting a balanced skin microbiome
- Providing antimicrobial protection against pathogens
- Helping defend against environmental stressors and UV damage
02—When Things Go Wrong
When sebaceous function becomes disrupted
Many common skin conditions—including acne, rosacea, dermatitis, and enlarged pores—involve disruptions in sebaceous gland activity. Interestingly, this does not always mean the skin is producing too much oil. In many cases, the problem lies in the quality of the oil and the signaling pathways within sebaceous cells.
Several factors influence how sebaceous glands behave:
- Hormonal fluctuations—androgens directly stimulate sebaceous gland activity and sebum production, which is why hormonal acne is so common
- Metabolic signals—insulin and IGF-1 influence sebaceous output, which is why diet can play a meaningful role in acne-prone skin
- Microbiome imbalance—disruption of the skin's bacterial ecosystem alters how sebum is processed within the follicle
- Chronic inflammation—inflammatory signaling changes sebum composition over time, contributing to congestion and breakouts
- Environmental stress—UV exposure, pollution, and temperature shifts all affect sebaceous regulation
In acne-prone skin, research shows that sebum composition changes—often including a reduction in protective fatty acids and an increase in oxidized lipids that drive inflammation within the follicle. Which means oil alone is rarely the true root cause of acne or congestion.
03—Why Stripping Backfires
Why oil-stripping treatments often make things worse
Many traditional acne and oily skin treatments are built around one goal: eliminate oil. Harsh cleansers, aggressive exfoliation, benzoyl peroxide, and overuse of acids are commonly used to suppress oil production. While these approaches may temporarily reduce breakouts, they frequently disrupt the skin barrier and impair the sebaceous gland's ability to regulate itself.
In clinic, we often see the same cycle play out:
Barrier disruption—aggressive stripping removes the lipid layer the skin relies on for protection
Inflammation—a compromised barrier triggers an immune response within the skin
Rebound oil production—the sebaceous gland compensates by producing more sebum to protect the skin
Congestion—imbalanced sebum thickens and accumulates within the follicle
More stripping—and the cycle continues, leaving the skin exhausted and increasingly reactive
Instead of solving the problem, these approaches often deepen it. The skin becomes reactive, dehydrated, and increasingly difficult to treat—not because the skin is broken, but because the strategy is working against it.

Why your pores look larger when sebum is imbalanced
One of the most common concerns clients come in with is the appearance of large pores. Pores themselves do not open and close—but their appearance can change significantly depending on what is happening inside the follicle.
When sebaceous glands produce imbalanced sebum, several things can occur:
- Oil thickens and becomes harder to flow through the follicle naturally
- Dead skin accumulates within the follicle lining
- Oxidation causes darker sebaceous filaments to become visible at the surface
- Chronic inflammation stretches the follicle wall over time
